Eligibility
##### *For Beneficiary (Person who underwent Sterilization)* - The applicant must be a person who has undergone sterilization operation under family planning program. - The applicant must have undergone sterilization operation at an authorized health facility. - The applicant must be able to provide documentary evidence of the sterilization procedure. ##### *For Claimant (Family Member or Legal Representative)* - The applicant must be a legal claimant or family member of the person who underwent sterilization (in case of death or incapacitation). - The applicant should have legal authority to represent the beneficiary in case of death or inability to claim. > *Conditions* _*The applicant must report complications, failure, or death within the specified timeframes as per scheme guidelines._ _*The applicant must submit the claim through proper medical channels with certification from authorized medical officers._
Benefits
Section A: Death following sterilization (inclusive of death during sterilization operation) in hospital or within 7 days from discharge - ₹4,00,000/-. Section B: Death following sterilization within 8-30 days from discharge - ₹1,00,000/-. Section C: Failure of sterilization - ₹60,000/-. Section D: Cost of treatment in hospital and up to 60 days arising out of complications following sterilization operation - Actual cost not exceeding ₹50,000/-. > *Conditions* _*The claimant must accept the compensation amount as full settlement with no further claim rights._ _*The claimant must warrant the truth of all particulars provided, with forfeiture of compensation for false statements._ _*The claimant must agree that the compensation covers all aspects of the claim with no additional benefits._
